Registration for U-18 Fall 2011 Rec Season -- Until August 31! |
MRYSL is forming U-18 teams -- must be between 14 years old and 18 years old, as of August 1st, 2011)
Complete registration includes:
o Two copies or duplicates of signed Registration Form 1601
o Verification of birth date (players who are new to MRYSL must provide copy of official birth certificate, Medi-Cal card, or passport)
o Check for registration and volunteer fees, made payable to MRYSL
o Send to: Mad River Youth Soccer League P.O. Box 103 Arcata, CA 95518
Registration must be postmarked by August 31, 2011

Deliver a complete registration packet (see the check list items above) to:

Fees for 2011:
First Child: $75.00 Second Child: $70.00 Each Additional Child: $65.00
Each family must also pay an additional $20.00 volunteer fee per family (not per child). The fee is refundable upon completion of two hours of volunteer time for the league.
Fees for unprocessed registrations will be returned by September 30, 2011
