Complete registration includes:
o Two copies or duplicates of signed Registration Form 1601
o Verification of birth date (players who are new to MRYSL must provide copy of official birth certificate, Medi-Cal card, or passport)
o Check for program fee, made payable to MRYSL

Fees for 2013: First Child: $105.00 Second Child: $75.00 Each Additional Child: $70.00.
Late Fee: $25.00
NEW this year: Registration fees can be paid via credit card, debit card or paypal account. Please see our PayPal page for more info.
A $25.00 rebate per family is available if you volunteer for 3 hours or more. Click HERE for info on our volunteer program.
A limited amount of financial aid may be available. Please check our Forms Page for application forms.

THREE WAYS to send in your complete registration packet:
(1) Mail to Mad River Youth Soccer League, P.O. Box 103, Arcata, CA 95518;
(2) Drop Off at MRYSL’s office, at 1309 11th St., Suite 202, Arcata. Office Hours: Tues, Thurs, Fri, 9:00 am to Noon
(Mail is not accepted at this address).
(3) Attend an In Person Registration Event:
Arcata: Thursday, May 23rd, 5 to 7 pm, Arcata Community Center lobby
Hoopa:Sun May 19th & Sun. May 26, 11 am to 2 pm Hoopa Soccer Field;
McKinleyville Sat. May 11 & Sat. May 18th, 10 am to Noon at Rays Food Place, 1500 Anna Sparks Way, McKinleyville.
