2009 Fall Registration Information & Instructions
Please follow Instructions Carefully.
Incomplete forms could jeopardize you child’s placement on a team.

Important General Instructions:
1. Please complete a 1601 registration form or print and fill out 2 copies from our website (see link below). Make sure you sign wherever a signature is needed. Please enter your email address in the appropriate space; we plan to start using email for most of our intra-league communication.
2. Please complete a separate registration form for each child.
3. New this year, we need the mother’s birth month and date for all players. The newer forms have a specific spot for this. On the older forms, please write the month and date next to the mother’s name.
4. Verification of birth date is required. All new players must show proof of birth date. Acceptable formats: birth certificate, passport, alien registration card, or Medi-cal card.

Coach/Team requests: We do our best to accommodate requests. Write your request on the form in the space marked “other” under Parental Support. There are no guarantees.

Team formation/Important dates: Teams are drafted in July. We try and form the teams as fairly as possible with limited information. Coaches will be given their team rosters in August at the coaches’ meeting. Practice begins as early as August 10th. The Jamboree (pictures and first games) is August 29th.

Fees: D4 First Child: $75 Second Child: $70 Each Additional Child: $65

Volunteer Policy: New this year! Due to lack of volunteers, we are adopting a new volunteer policy. Families will pay a $20 volunteer fee with registration. This way the league can afford to pay for such things as lining fields, field preparation, etc. The volunteer fee may be earned back by volunteering 2 hours during the season.

Late registration: Teams are formed first come, first served. There will be $25.00 late fee form registration postmarked after May 31st. After May 31st, players will be placed on a waiting list and notified of team placement depending on availability. Fees will be returned by September 30, 2009.

Payments: Make check or money order payable to MRYSL. If your check bounces, you will be charged a $10 service fee. Your player will be placed on the waiting list until fee payments are resolved.

Refunds: The only way to receive a refund is to notify the league in writing by August 31st. If your child was already placed on a team, there is a $25.00 service fee deducted from the refund. Only acceptable reasons for a refund: child becomes ill or injured & is unable to participate in games or moving out of the area prior to season.

Financial Assistance: To be eligible, your registration must be postmarked by May 31, 2009.
